He called the Help line today and they have no idea how to help him. The stated that Playonline is not Square-Enix, and they have no control over the problem.....
I just did web chat with someone instead of calling. All I had to do was give them my SE username, e-mail I signed up with, and my birthdate and they restored the account for me. I did the exact same thing and got locked out. All fixed now.
